With over 26 years of experience, Susan A. Schaffer is a Licensed Clinical Social Worker who is dedicated to empowering individuals on their journey to mental well-being. Susan graduated with a Master's Degree in Social Work from Loyola University Chicago in 1998 & earned a Bachelor's Degree in Psychology in 1996. Her graduate clinical training included counseling and care management services at the Council for Jewish Elderly and Community Counseling Centers of Chicago (C4). Susan has provided direct care services to individual diagnosed with a Severe Mental Illness in a Community Integrated Living Arrangement (CILA) and a Linkage Case Management program serving individuals post-discharge from hospital settings: counseling, coordinating, linking, & supporting individuals in a community-based setting. For the past 20 years, Susan has supervised case management programs which have successfully assisted & supported individuals diagnosed with a Severe Mental Illness and/or complicated with medical needs. Susan was the Director for a Pre-Admission Screening/Mental Health (PAS/MH) process from 2000-2022 and Director for Outreach & Assessments for the Colbert & Williams Consent Decrees (2012-2024).
