Susan A. Schaffer is a Licensed Clinical Social Worker with 24 years of experience working with adults diagnosed with mental illness and/or complex co-morbidities. Susan graduated with a Master's Degree in Social Work in 1998 from Loyola University Chicago, School of Social Work and earned Bachelor's degree in Psychology from Loyola University Chicago in 1996. Her graduate clinical training included counseling and care management services at the Council for Jewish Elderly and at the Community Counseling Centers of Chicago (C4). Susan has provided direct care services to individuals diagnosed with a Severe Mental Illness in a Community Integrated Living Arrangement (CILA) program and also with a Linkage Case Management program serving individuals post-discharge from hospital settings; counseling, coordinating, linking and supporting individuals in a community-based setting. For the past 20 years, Susan has supervised case management programs which have successfully assisted and supported individuals diagnosed with a severe mental illness in the community. Currently, Susan is the Director for the Pre-Admission Screening/Mental Health (PAS/MH) process for individuals diagnosed with a Severe Mental Illness seeking nursing level of care and also Director for Outreach and Assessment services offered through the Comprehensive Class Member Transition program at a non-profit organization in Chicago.
